How to Join:

Complete the membership application online below or download a paper application.
Pay online once you have received an invoice via email, or mail check to 3121 Eastern Avenue, Baltimore, MD 21224.
If your organization requires an IRS Form W-9 to process payment, you can download it here.

Organizational Membership - $300.00 (USD) Bundle (up to 4 members) Subscription period: 1 year No automatically recurring payments Organization and staff information will be added to our membership list and website, up to (3) additional staff members can be added to the membership online once membership fee has been paid and membership is activated.

Please note: the bundle coordinator counts as one of the four (4) members.

Instructions for how to add members to the membership will be emailed to the bundle coordinator once the membership is activated.

Organizations that wish to add additional members beyond the four (4) included can do so for an additional fee of $50.00 per person. This fee can be paid online and then please email contact@lpnmd.org with additional staff's name and contact information to be added to the membership.

Individual Membership - $125.00 (USD) Subscription period: 1 year No automatically recurring payments Individual and organization they represent will be added to our membership list and website

Student Membership (undergraduate students only) - $25.00 (USD) Subscription period: 1 year No automatically recurring payments Student members are required to provide 10 volunteer hours at LPN-sponsored events in addition to the annual fee; must provide proof of undergraduate status and use school-issued email address when applying.
